Submitting a publication request

• Allow adequate time for design, editing/proofing, and printing. In general, smaller jobs require two weeks depending on the complexity of the job.
• Include written content for publication when submitting request. (attach at the bottom of this form)
• Make arrangements for necessary mailing services.
